Durable Medical Equipment
PROVIDE A CARE can arrange for rental and purchase of wheel chairs, walkers, special beds, diabetic medical supplies, or any medical assistive devices which may be needed.

Laboratory Services
Physician ordered tests which can be performed at home, such as urinalysis and blood tests. Results are immediately reported to the physician.

Additional Services
Upon your request PROVIDE A CARE can refer physicians and podiatrists who can visit your home.

ELIGIBILITY
To qualify for care from PROVIDE A CARE a patient must:

• Be essentially homebound, Need not be bedridden but needs assistance to leave home
• Need intermittent care
• Require skilled medical care by healthcare professionals

All patients are accepted for care regardless of race, color, sex, national origin, religion, age or disability. MEDICARE, Medicaid and commercial insurances pay for home health services.
